Our Dedicated Team
A Touch of Grace Hospice provides care through an interdisciplinary team made up of physicians, nurses, hospice aides, homemakers, a social worker and a chaplain. The combined expertise in these disciplines allows A Touch of Grace Hospice to provide excellent care to our patients and their families.

Our Physicians are licensed, experienced and knowledgeable in the areas of hospice care, pain management and symptom control. Our physicians oversee the entire interdisciplinary team and collaborate as needed with the attending physician to ensure the patient maintains a comfortable and excellent quality of life as a hospice patient.

Our Hospice Nurses have many years of experience working in the nursing field, including significant experience as hospice nurses. This experience has equipped them with the ability to effectively manage pain and other symptoms our patients often experience. All nursing care is provided under the guidance of the patient’s attending physician, who is routinely informed of the patient’s condition.

The Hospice Aides at A Touch of Grace Hospice, are registered as qualified aides with the state. They are compassionate and display patience when providing personal care to our patients, which includes bathing, dressing, grooming and feeding patients as needed.

Our licensed, masters-level Social Workers have a vast knowledge of community resources, counseling and patient advocacy. They work closely with patients and families to assist them with various social needs that range from family counseling to resource referrals. They also help determine if patients and families qualify for other services in the community, and if so, they work with the family to help access these services.

The masters-level Chaplains who provide spiritual support to our patients and families work closely with those they serve to help address spiritual concerns that impact quality of life for the entire family unit. They assist with Celebration of Life Services, which are memorial services held in honor of the patient after his or her death. In addition, our Chaplains provide bereavement support to families for 12 months following the death of a loved one.

Our Homemakers are available to assist patients and families as needed with duties such as light housekeeping, cooking, laundry or providing a respite for the primary caregiver. Homemaker services are also offered through other community agencies, and the hospice social worker assesses whether homemaker services will be provided by A Touch of Grace Hospice or another community agency.
